Age 66 of Brook Park, passed away peacefully on November 21, 2025.

Jack is survived in life by Gail, his beloved wife of 48 years.

Devoted Father of Jack Jr. (Kristy) Lezak, James Lezak, and Jason Lezak.

Loving Brother of Susan Gibbs, Harry Lezak, Darlene (Joe) Pellrite, Terry (Debra) Lezak, and Michael (Carole) Lezak.

Cherished Grandfather of Paige and Peyton.

Dear Uncle too many nieces and nephews, and a friend to many.

Jack loved all animals, especially his own, their names our Bella, Ollie, Ozzie, Harvey, Gracie, Emmy, Jake, and Molly.

The family will receive friends in celebration of Jack’s life at Humenik Funeral Chapel 14200 Snow Rd Brook Park, 44142, on Sunday, December 14, from 2-5 PM, where Funeral Services will be at 4:30 PM.
